The first visit will include:

- Prescription of a home exercise program and education/ advice

- Practice of exercises

- Discussion of plan of care

What to bring?

In order to prepare for your first appointment, please fill out the required paperwork in advance. You will find it on the Patient Forms page. Remember to mail it to us or bring it with you to your first scheduled visit.

Please make sure to also bring:

- Your referral for physical therapy, if required by your insurance for reimbursement

- The relevant diagnostic reports you may have (X-Rays, MRI's, etc.)

- A photo ID, your insurance card, and your preferred form of payment (we accept cash and checks, no cards)

- Comfortable, loose-fitting clothing that allows unobstructed access to the affected body region (this may include a

pair of shorts and a tank top or a swim suit)

- Your footwear and/or orthotics, if relevant

How long will it take?

Physical therapy sessions typically last 1 hour. However, the first visit usually lasts longer (75-90 minutes), depending on your specific needs. Please arrive on time. Please note that if you arrive late for your appointment, your time with the therapist will be shortened in order to avoid delaying those patients scheduled after you.
